They should be under anesthetic because, otherwise, you're cutting on a cat that's awake. I’m ot trying to be silly, but to answer that question, yes. I would think if it is a true surgery, then that cat should be under anesthesia.

There would be some type of anesthesia for surgical procedures, whether injectable or inhalant anesthesia, because we wouldn't want them to feel the pain of that procedure.

Yes, your cat will be under anesthesia during the surgery. Unfortunately, we can't use local anesthetics on pets as we can't ask them to hold still.
